HELLP Syndrome – Fiche Synthèse & Algorithme

HELLP SYNDROME – SYNTHÈSE PRATIQUE

D’après CNGOF 2020 (actualisation 2025)
DÉFINITION

Triade biologique : Hémolyse + Élévation des transaminases + Thrombopénie.
Survient chez une patiente prééclamptique (ou parfois sans HTA).

SIGNES D’APPEL
  • Douleur épigastrique / hypochondre droit (70–80 %)
  • Nausées, vomissements (30–50 %)
  • Signes de toxémie sévère : céphalées, phosphènes
  • Ictère rare (5 %)
BIOLOGIE – SEUILS CLÉS
Hémolyse
  • LDH > 600 UI/L
  • Haptoglobine effondree
  • Schizocytes (inconstant)
Cytolyse hépatique
  • ASAT > 70 UI/L
  • ALAT > 40–50 UI/L
Thrombopénie
  • < 100 000/mm³
  • Sévère si < 50 000 | Modérée si 50 000 – 100 000/mm³
GRAVITÉ – CLASSIFICATION CNGOF
● SÉVÈRE (1 critère suffit)
  • Plaquettes < 50 000
  • ASAT/ALAT > 3N
  • HTA sévère (PAS ≥160 / PAD ≥110)
  • Signes fonctionnels intenses
  • IRA, oligurie
  • RCF pathologique
  • Hématome sous-capsulaire
● MODÉRÉE
  • Plaquettes ≥ 50 000
  • ASAT/ALAT < 3N
  • Pas de signes de gravité
PRISE EN CHARGE OBSTÉTRICALE
AG ≥ 34 SA → Extraction fœtale RAPIDE (césarienne le plus souvent)
AG < 34 SA ET HELLP SÉVÈRE → Césarienne IMMÉDIATE
AG < 34 SA ET HELLP MODÉRÉ → Corticothérapie + expectative 48h max
TRAITEMENTS – CE QU’IL FAUT SAVOIR
  • Corticothérapie anténatale : UNIQUEMENT si AG < 34 SA (pour le fœtus) – PAS pour traiter le HELLP
  • Transfusion plaquettes : césarienne si plaquettes < 50 000 / voie basse si < 20–30 000
  • Sulfate de magnésium : si prééclampsie sévère associée
  • Valaciclovir, corticoïdes dose thérapeutique : PAS d’indication
SURVEILLANCE POST-PARTUM
Phase aiguë (0–48h)

PA, NFS, ASAT/ALAT, LDH, créatinine, diurèse toutes les 6–12h

J7 post-partum

PA, NFS, ASAT/ALAT, LDH systématique.
Créatinine UNIQUEMENT si anormale à la sortie ou IR persistante

6 semaines (obligatoire)

PA + NFS + ASAT/ALAT + LDH + créatinine + protéinurie

COMPLICATIONS
  • Hématome sous-capsulaire du foie (1–5 %) → échographie hépatique
  • Rupture hépatique (< 1 %) → scanner, chirurgie
  • IRA (15–30 %) → surveillance créatinine
  • CIVD (15–20 %) → TQ/TCA, fibrinogène
  • OAP (5–10 %) / Hématome rétroplacentaire (10–20 %)
RÉCURRENCE
Risque global

25 %

Si HELLP avant 32 SA

60 %

Risque prééclampsie ultérieure

30–40 %

DIAGNOSTIC DIFFÉRENTIEL : HELLP vs SHAG
Critère HELLP SHAG
TQ/TCA Normaux Allongés
Fibrinogène Normal Diminué
Hypoglycémie Rare Fréquente
Bilirubine ++ (< 5 mg/dL) +++ (> 5 mg/dL)
Créatinine + (20–30 %) +++ (80–90 %)
